2025 Community Capacity-Building Funding for HRSN services

The Oregon Health Authority (OHA) will be distributing Community Capacity Building Funding (CCBF) to coordinated care organizations (CCOs) interested in providing Health-Related Social Needs (HRSN) services such as medically necessary climate-related home remediations, housing-related supports, and nutrition-related supports.

Available funding

The four CCO regions have the following amounts of CCBF funding available in 2025:

  • Lane County: $3,822,083
  • Marion and Polk counties: $5,581,652
  • Central Oregon: $3,260,088
  • Columbia Gorge: $1,271,206

Priority funding areas

The priority areas of funding for all CCO regions include: 

  • Increasing the capacity of HRSN housing service providers, with a focus on providers who can accept external HRSN housing requests
  • Building local HRSN service provider networks that can support priority populations—which may include Latinx, LGBTQIA2S+, Black/African American, Slavic/Eastern European, and other communities—in each of the four CCO regions
  • Expanding local HRSN nutrition services for medically tailored meals and other future HRSN nutrition services

How to apply

Applications for PacificSource CCBF funding will be accepted from April 1, 2025, to May 30, 2025.

To apply for CCBF, you’ll need to complete the CCBF application (PDF) and budget request form (XLS). Then, you’ll submit these materials in our grant portal below.

Application evaluation process

After the application window closes, PacificSource will conduct four separate reviews, one for each CCO region. Review teams will include PacificSource staff and community partners.

We’ll look for strong applications that support priority funding areas and demonstrate that the CCBF awards will be used to quickly start providing HRSN services.

During the review process, we may contact other CCOs and applicants when necessary to clarify application details and award amounts. 

Successful applicants are tentatively scheduled to receive letters of award by October 2025.
